REGISTRATION
WHAT FORMS DO I NEED TO BRING TO THE SCHOOL?
1. ORIGINAL copy of child's birth certificate
2. Proof of residency (something with name and address i.e .lease, phone bill, electric bill, etc.)
3. Custody papers if applicable (must have the seal)
OTHER INFORMATION TO BRING:
1. Grades from previous school
2. Standardized test scores
3. If your child receives special services we will need a copy of their current IEP (individual education plan)and MFE (multi-factor evaluation)
4. Immunization Records
INFORMATION TO FILL OUT AT SCHOOL:
1. Enrollment data sheet
2. Emergency medical form (anyone that you want to authorize to pick your child up from school must be listed with name, address, and phone number)

*These must be paid in order for you to receive your child's quarter report card. If this is a problem, you should talk with the building principal to set up a payment schedule.
