Boy, if I've heard that once in the last week, I've heard it two hundred times! Final Forms is the new data collection for the Mariemont City Schools, and in the long run, it will be a time saver and an aggrevation rudecer for the parents, students, and coaches of the District. As I parent, I know there are many times when I'm unsure if I have completed a form for my child, or if there's an online account invovled, which of my seven passwords have I used to log in with. And then after I call the school, well, actually, I just call another office since I work here, I find out it was the 8th password that I only use for school stuff that I couldn't remember. I've been there with you!
So, as you have set up your Final Forms account and enter all of your student's information, and completed the waivers, and the emergency forms, and the Code of Conduct, and watched the OHSAA video, all information that you need to know. Final Forms will keep a record of that, and you don't have to fill out any paper. And when you need that information for next year, it's there and you will just have to verify. Remember how you have to fill out the physical form EVERY year, and answer the same questions? Final Forms will keep that record for you and you just double check it next year. And after you answer those physical questions, Final Forms will tell you to print the sheet for your doctor for the physical. After the physical, you bring the form to the office and we will record it. And next year, as your physical expires, it will give you reminders at 60 days before, 30 days before, and then a countdown from 10 days. It's always good to have someone reminding us. But it's been a busy week. We've been tracking physicals manually this week, but they will be in your account early next week.
